All nominees must live or undertake their volunteering work in Cornwall. The closing date for nominations is Monday 14th September 2015.

The selection panel will choose a winner of each award. The decision will be based on the information included in the nomination form and the advice of local professionals e.g. County/Regional National Governing Body officials, Tempus Leisure Officers.

1. Children's Coach of the Year

The winner of this award will coach/instruct children under the age of 16; they will display development of basic skills through fun in sport and appropriate player-centred engagement. There will be evidence of high quality coaching/instruction and innovative practice.

2. Community Coach of the Year

The winner of this award will display the outstanding contribution of a coach/instructor getting more people starting in sport, staying in sport and/or succeeding in sport. There will be evidence of the impact of high quality coaching/instruction, innovative practice and working within a specific community or target group.

3. Young Coach of the Year - U25

The winner of this award will display dedication, passion, determination and enthusiasm as an up and coming coach/instructor. They will have made a contribution and commitment to players/athletes, together with demonstrating personal maturity, evidence of high quality coaching/instruction and innovative practice. (They will be age 25 or under on 30th September 2015).

4. Disability Coach of the Year

This award recognises the achievements of a coach/instructor working to develop disabled athletes at any level within sport. The evidence will highlight the impact the coach/instructor has had on widening access to sport for disabled people. There will be evidence of providing opportunities and/or dedicated support within a mainstream sporting environment or a dedicated disability sport environment.

5. Performance & Development Coach of the Year

The winner of this award will recognise the achievements of a coach/instructor working to develop emerging and young performers within their sport. Evidence of player centred high quality coaching/instruction that encourages the development of performance and nurturing talent. As a specialist area, this could be a paid position.

Winners from this category will be put forward to the UK Coaching Awards organised by Sports Coach UK.

6. Official of the Year

The winner of this award will demonstrate a commitment as an official, umpire or referee together with commitment to their own personal development. There will be evidence of development, achievement and leading by example.

7. Club of the Year - Adult

The winner of this award will excel in the delivery of an overall sporting experience within a club environment. The club will have evidence of social and community development, together with a commitment to club/volunteer development and increasing adult participation.

8. Club of the Year - Junior

The winner of this award will excel in the delivery of an overall sporting experience within a club environment. The club will have created links with schools and be open to the whole community. The club will also be committed to club and volunteer development.

9. Lifetime Services to Sport

The winner of this award will be recognised for their outstanding continued services to sport. They will demonstrate a commitment to volunteering and will have a particular enthusiasm and dedication to their sport. They will also have widened opportunities for participation in sport. Please also list any evidence of projects/areas they have worked on.

10. Community Volunteer of the Year

The winner of this award will be recognised for their outstanding contribution to the community. They will demonstrate a keen interest in their community and will have a particular enthusiasm and dedication to their sport. They will also show a commitment to club development.

11. Community Volunteer of the Year - U25

The winner of this award will be recognised for their work in the community. They will demonstrate personal maturity and a particular enthusiasm and dedication to their sport, together with a commitment to club development.

12. Young Leader of the Year

Must be registered on a Leadership course at school/college/club/community group

The winner of this award will have excelled in their volunteering and made a commitment to their own personal development. They will have made an outstanding contribution to Leadership and will have worked in school, community club or community environment.

Winners from the Club and Volunteering Category will be put forward to the BBC South West Awards where the awards are aligned.

13. Community Participation Project of the Year

This award is aimed at any organisation (community club, leisure/community centre, governing body, fitness professional, charity or workplace) that has delivered a project to get more people and ideally those aged over 14 into sport and physical activity. The winner will be able to demonstrate how they have marketed their project, engaged with the target audience and sustained them into community sport and physical activity. Examples of why the project was successful and numbers engaged with would support the nomination.

14. Active Workplace of the Year

The successful workplace will be able to demonstrate how they have 'gone the extra mile' to support their workforce to become more active by giving them opportunities pre, during and post work. The winner will also be registered with the counties Healthy Workplace Award and will be able to demonstrate evidence of achieving this or working towards this award.
