Par Track - Social Prescribing Facilitator

An exciting opportunity has arisen for someone is passionate about health promotion and healthy lifestyles who can build, develop and deliver a scheme to get the local community active.
Par Track have been successful in receiving project funding of £200k from Big Lottery Funding, Heritage Lottery Funding & NESTA, to run a Social Prescribing scheme from our Track. The long term
aim is to get the local and surround area to improve their Health & Wellbeing.
The successful candidate will develop a Social Prescribing programme which takes the holistic approach of supporting and improving the health and wellbeing of target groups and individuals,
through ensuring that the interventions best meet their tailored goals. The project will involve working with all age groups, including the local primary school, with the aim to get the community participating in regular health & wellbeing activity. The role will involve designing and delivering elements of activity.
The role will ensure that primary care services within the immediate vicinity and surrounding area are involved, engaged and utilise the social prescription model supporting their patients to take
responsibility for improving their health and wellbeing.
Applicants must hold a Level 3 qualification in Exercise Referral as a minimum and have experience of working with people of all ages, with proven experience of developing, promoting and implementing
physical activity and enrichment programmes; including experience of costing and budgeting.
